How to Register
Register with us
If you would like to register with our practice, please fill in our patient registration form
which may be obtained from the main reception at the practice.
Alternatively, you may complete this online by clicking on the links below.
We will ask you for:
- Proof of identity: ideally a passport, driving license or other photographic ID
- Recent proof of address i.e. a recent household bill or bank statement

Out of Area
Since 2015 we have been able to register patients who live outside the practice area but without any obligation to provide home visits.
Patients may approach any GP practice, even if they live outside the practice area, to see if they will be accepted on to the patient list.
Out of area registration is voluntary for GP practices meaning patients may be refused because they live out of area.
We may decide that it is not in your best interests or practical for you to be registered in this way. In these circumstances, we will not register you and will advise you to register (or remain registered) with a more local practice.
If accepted but your health needs change we may review your registration to see if it would be more appropriate for you to be registered with a GP practice closer to your home.

Once you have been accepted as a patient...
Once you have been accepted as a patient, your medical records will be transferred to our practice and you will be informed about this. Your medical records may take several weeks to arrive at the practice.
If you are on regular medication, please make a 20-minute appointment once you have signed up. This will allow our doctors to stay on top of your health care needs.
We advise all our new patients to make a routine health check with our practice Health Care Assistants within the first three months of joining our practice.
The practice has a non-discrimination policy for accepting new patients. There are several reasons why you may not be able to register with your chosen GP. For example, you may live too far away. If this is the case, simply choose another GP in your local area.
If English is not your first language, the practice has access to interpreters. Please inform reception staff and this assistance can be provided.
Proxy access

New and current patients can book appointments, order repeat prescriptions and see information in your GP records, including medications and allergies online.
Proxy access. What is Proxy access?
As a parent, family member or carer, you may be able to access services for someone else. This is called having proxy access.
Examples include:
- A parent or guardian who has legal responsibility for a patient under 11
- A carer for a patient over the age of 16 years. A letter from the patient is required giving them permission.
A proxy (someone managing another person's records) does not need to be registered as a patient at the practice but must have their own online service login.

- Under 11: A parent or guardian with legal responsibility can request proxy access.
- Ages 11-16: Proxy access is automatically limited when a child turns 11. The GP may assess whether the child can consent to share their records. From ages 11 to 16, parents can manage some aspects, such as booking appointments or ordering prescriptions, but will not see past appointments or clinical records.
- Ages 16 and above: Proxy access is removed when the patient turns 16 unless the young person consents to continue it. They can request to reinstate proxy access by visiting the practice with proof of ID.
Linked profiles in your NHS account
Once proxy access is set up, you can access the other person’s profile in your NHS account, using the NHS App or website.
The NHS website has information about using linked profiles to access services for someone else.
When Proxy Access May Be Refused or Removed
The practice may refuse or withdraw proxy access if it is deemed to be against the patient’s best interests.

Disabled Patient Facilities
Disabled patient facilities are also available at this practice.
