Social Prescribers are here to help!

Springfield Medical Centre is proud to have a dedicated social prescriber as part of the team.

Social prescribing is a way for local agencies to refer people to a link worker. Link workers give people time, focusing on ‘what matters to me’ and taking a holistic approach to people’s health and wellbeing. They connect people to community groups and statutory services for practical and emotional support.

Social prescribing works for a wide range of people, including people:

  • with one or more long-term conditions
  • who need support with their mental health
  • who are lonely or isolated
  • who have complex social needs which affect their wellbeing

Our dedicated social prescriber is Ms Ana Rei.

Ana has been as a Social Prescriber since December 2019. Prior to that Ana worked for Respeito supporting vulnerable families and victims of Domestic Violence. She participates in the Lambeth Wellbeing Portuguese Partnership actively and previously worked as Mental Health and Wellbeing Project Coordinator for Latin American Disabled People Project which continuously support  and advocates for the integration of Spanish and Portuguese speakers residents across London to access health  and welfare services. Ana is also a Yoga Practitioner for the last 10 years and can share knowledge of own wellbeing practices like mindfulness & positive thinking. 


Ana Rei has been working as a Social Prescriber with Springfield Medical Centre since August 2020. In this role she has been guiding people in remaining active by joining local activities, support them with benefits, referral to social services for care needs and equipment assessments and a lot more.

Social Clubs and Activities in August

With summer in full swing, we’ve been enjoying this sunny weather and the opportunity to run clubs outside. The easing of restrictions mean we are also now running face-to-face clubs indoors. However, in order to keep everybody safe and comfortable, there will be limited places and COVID-19 safety measures at every club.
