If you are diagnosed with hypertension, you should have an annual review with a Nurse or Healthcare Assistant.
“Every year during your birthday month, you will receive a text to invite you to a hypertension review. Then you can call our reception to book an appointment. During the review, I will:
Do a blood test
Ask for a urine sample
Measure blood pressure
Take the pulse
Measure your height and weight
I will also ask whether you are taking the medication regularly and if there are any questions you would like to discuss.”
